Our Story: Care Rooted in Experience
Dalu Lewis's career has been defined by listening to families and patients at some of their most uncertain moments. Her nursing journey began over twenty years ago as a Licensed Practical Nurse, where she witnessed first-hand how overwhelming healthcare transitions can be. Driven by a deep sense of responsibility and a desire to give families clearer direction, Dalu continued her education, earning an Associate degree, then a Bachelor of Science in Nursing, and ultimately a Master's degree. With board certifications in both Family Nurse Practice (FNP-BC) and Hospice and Palliative Care (ACHPN), she built a unique skillset grounded in compassion and clinical excellence.
Her roles have spanned skilled nursing facilities, hospitals, and community care, helping individuals at every stage of illness receive support tailored to their true needs. Again and again, she noticed families expressing the same challenges: confusion about next steps, frustration with conflicting information, and fear of missing important resources or losing control of care choices. This clarity gap inspired her to create Impact Palliative Care Navigator - a place where real-world experience meets heartfelt advocacy. Dalu's mission is to make the care journey less intimidating by standing with families and helping them find their footing, one conversation at a time. By translating complex healthcare systems into understandable steps, she empowers clients to make confident choices. With over 20 years of nursing experience, working across a wide range of healthcare settings, including hospitals, skilled nursing facilities, and community and home-based care. Having Board-Certifications as a Family Nurse Practitioner and Board-Certified in Hospice and Palliative Care, the years of experience allows compassionate, knowledgeable, and patient- centered support tailored to each individual and family's unique needs.
